Commonwealth of PA Death Certificate Registration

The death certificate is the legal record of the fact of death of an individual. As a permanent legal record, the death certificate is extremely important to the decedent's family. Statistical data from death certificates are used to identify public health problems and measure the results of programs established to alleviate these problems.

The filing of the death certificate is the responsibility of the funeral director or person acting as such; however, others share the responsibility for the completion of the death certificate.
